Expectations
It is recognized that the optimal model or structure for public health will vary from one community to another, depending on local needs, resources and readiness.
All models, however, are intended to respect and accommodate local differences; seek to integrate public and private capacity for public health services; and adequately protect the publicıs health. All public health partnerships are tasked with assuring provision of the
Ten Essential Services of Public Health. Public health partnerships are engaged in a range of
activities that promote and protect the health and safety of NH residents.
Expectations and Requirements
- Engage broad-based partnership in the local public health system
- Conduct public health infrastructure assessment identifying strengths & weaknesses
- Conduct community needs assessment identifying priority health concerns
- Facilitate community-wide effort to identify high priority issues for health improvement
- Produce Public Health Improvement Plan detailing priorities for action
- Assure an effective community-based public health emergency response capability
- Establish communications and formal coordination of essential services with state agencies
